This allows you to see if we are a good fit for you & your child, and we can assess the level they will fit best in.

Consent & Acknowledgement

1. I understand that swimming and water activities carry inherent risks, and while every precaution is taken to ensure safety, Azure Dream Enterprise T/A Azure Swim Academy, its instructors, and staff cannot be held liable for any injury, illness, accident, or loss of personal property that may occur.

2. I acknowledge that classes are conducted under the supervision of trained instructors, and safety protocols are in place.

3. I accept responsibility for notifying the instructors of any medical conditions, allergies, or special needs that may affect my child’s participation.
